Health and Care is Changing in Leeds. We are delivering a new approach to service planning and design, where services are built around people, communities and neighbourhoods
Leeds has a strong health and care system, but we know it doesn't always feel joined up, easy to access or focused on what matters most to people.
That's why health and care organisations across Leeds are working together to create a new way of delivering support called Neighbourhood Health. This approach will bring services closer to where people live, strengthen community support, and help people stay healthy and independent for longer.
Neighbourhood Health is not a new service. It is a new way of working together across health, social care, the council, community organisations and other local partners to improve people's lives.
