Vision for the programme
We are working with our partners in the city to improve (transform) the way community mental health services are delivered in Leeds.
This project is about bringing together all the services that support people with mental health needs. This ensures that people get the right care, in the right place, without having to repeat their story.
Supporting primary and community mental health services will mean that we can:
- Respond better to people’s needs
- Remove barriers to access
- Support people to support care and treatment as early as possible
- To live as well as possible in their communities
Our new model of care has been developed and is being delivered with system partners, in line with ambition set out in the NHS Mental Health Implementation Plan (2019-2023/24) and the associated NHS England Community Mental Health Roadmap (2023).
