Central Local Care Partnership

Central Local Care Partnership is the geographical footprint that covers Central North including areas of north Leeds including Alwoodley, Shadwell, Oakwood, Roundhay and into Meanwood.

Our partnership includes local GPs (see below for list of GP surgeries involved), community health services, adult social care, local councillors, pharmacy, third sector organisations and community groups. We are keen for all providers of health and wellbeing support in our area to engage with us, so that we can work with one another to support people in our local area. We are still determining our full list of priorities, with ageing well being the priority we can best make a difference by working together.

Current priorities

Ageing well (population health management)

We are working together to understand where the greatest opportunities are to improve health and wellbeing for defined groups of local people. By using data and a population health management approach, we are supporting a group of local people who are living with frailty and who are housebound.

As part of the programme, a care coordinator role will be involved in proactive, person-centred care planning with this group of people – finding out what is important to them and overseeing and coordinating their care when needed. Linking with existing services and initiatives within the local area will be key, as well as exploring how local resources can be used differently or redesigned to make the best use of collective resources.
